The scan opens the browser instead of the Instagram app.
That's expected and normal. A QR encodes a web link, and each phone decides whether to open it in the app or the browser. No QR can force the app on every device. If the app is installed, Instagram usually hands the profile off to it. If not, the web profile still shows your Follow button.
The code goes to the wrong profile or a 'user not found' page.
Check the username is spelled exactly as on Instagram: handles are case-insensitive but every character counts, including dots and underscores. If you pasted a URL, make sure it's your profile link (instagram.com/yourhandle), not a link to a single post or a story.
The code scans but nothing happens.
Use the phone's built-in camera or a mainstream QR app, ensure the whole code is in frame with its white margin, and test under the lighting where it will live. If you added a large logo, raise error correction to Q or H so more of the code survives being covered.
