The QR opens Safari to a web page instead of the App Store app.
That's expected on the web, and on iPhone/iPad an apps.apple.com link opens the App Store app automatically. Make sure you pasted the full apps.apple.com URL for your listing (not a short marketing redirect that could be blocked), and test by scanning with the iOS Camera app, which hands the link to the App Store.
My branded, colored code won't scan.
Check the live scan-test badge. Usual causes are low contrast (light code on a light background), too small a quiet-zone margin, or an app icon covering too much of the code. Keep a dark code on a light background, hold the margin, shrink the icon, and set error correction to H when a logo is present.
The exported image looks blurry when printed.
Export a vector format (SVG, PDF or EPS) which scales to any physical size with no pixelation, or raise the raster size to 2048 or 4096px. A small PNG stretched to poster or packaging size will always look soft.
